About this template

For HVAC, plumbing, lawn care, pest control and pool companies that sell yearly maintenance plans. See whether your plan makes money on its own and with the repair work members bring, track every member's renewal date, reminder window and visits due, and send the agreement, sign-up consent, renewal reminders, price-change notices and cancellation confirmations from one kit.

A maintenance plan is steady income only if the price covers the visits. The Plan Pricing sheet takes your own technician cost, visit time, truck and materials, admin and card fees, and shows what one member costs you a year, whether the plan makes money on its own, what it earns counting the repair work members bring, the break-even price and the price at your target margin. The Members sheet gives every agreement its renewal date, the window for its renewal reminder, the visits due this season and a status, and the Notices log checks each reminder against the window your state sets.

The paperwork: a maintenance plan agreement that says plainly what the plan does not cover, a sign-up form with a separate automatic-renewal consent, a renewal reminder with a box for changed terms, a price-change notice, a cancellation confirmation, a visit record, a technician offer card and a reviewer checklist. In some states a plan that pays for repairs or guarantees equipment is regulated as a service contract or warranty, so the agreement keeps those out and the checklist names the questions for your reviewer. The completed example is a nine-person HVAC company in North Carolina with 119 active members.
